How To Initiate An LRA Proposal

Certainly you are welcome to call me and discuss a project at any time.   Jointly, we will complete the following Request for Proposal on the phone or you may forward it to me by email. The purpose of the RFP is twofold:

• To assist you in solidifying your thoughts, and,

• To provide a template upon which I can base a proposal.

Please have a look at the following checklist and consider some of its requirements.  If you wish to copy it into an email and send the answers to me, all the better.

Proposal Initiation Checklist

  • Organization
  • Address
  • City / State/ Zip
  • Phone
  • Fax
  • Email Address
  • Generic Description of Commodity or Service Required
    • i.e., What is it for? What does it do? Attach descriptive literature if available
  • Typical Brand Names and Serial or Style Numbers (if applicable):
  • Names of Suppliers/Sources/Providers — Name / Address / City / Phone / Fax / Contact Person
    • Attach complete mailing list of all potential suppliers if possible
  • Initial Cost Estimate (Each Item or Unit):
  • Quantity Required:
  • Total Cost Estimate:
  • Bidder/Vendor Response Needed By (Date):
  • Proposed Purchase Date:
  • Source of Funds — Grant, Budget Funds Released, Supplemental Budget, Levy, Other
    • Indicate if procurement is not funded at this time
  • Recommended Publications For Advertisement Of This Solicitation
    • Not local newspapers unless necessary; show industry periodicals, journals, etc.
  • Attach Performance Specifications if they are available at this time —
    • If possible, please supply specifications on disc in WordPerfect or MS Word
